* Are you a seasoned RN ready to move from the bedside to a high-impact leadership role? As an
** RN Case Manager** , you are the central hub of patient care. You will navigate the complexities of utilization management, patient throughput, and discharge planning to ensure every patient receives the right care at the right time. Reporting to the Director of Case Management, you'll be a key voice in interdisciplinary teams, advocating for patient resources and clinical efficiency.

** Base Pay:** $31.94 (Min) | $39.93 (10+ yrs) | $43.92 (15+ yrs)

** Evening Differential:** +$ 2.50/hr

** Weekend Differential:** +$ 2.00/hr

** Strategic Coordination:
** Conduct medical and psychosocial assessments to develop comprehensive, individualized discharge plans.

** Utilization Expert:
** Evaluate admissions for medical necessity and collaborate with third-party payors to ensure seamless coverage.

** Barrier Buster:
** Identify and resolve bottlenecks in patient throughput, ensuring high-quality outcomes and efficient bed management.

** Patient Advocate:
** Serve as the primary liaison between physicians, families, and community resources, specifically supporting high-risk and underserved populations.

** Process Leader:
** Participate in performance improvement initiatives to refine how our hospital delivers care.
** Qualifications*

* Experience:
** 3+ years of recent
** Acute Care Case Management
** experience is ideal.
+ _Will also consider:_ RNs with 3+ years in Med/Surg, Tele, ICU, or ER.
+ _Special
Note:
_ Candidates with Home Health or Insurance CM experience are welcome if they have 3 years of total Acute Care experience.

** Licensure:
** Active FL RN license (or Compact with the ability to obtain FL license).

* Education:
** ADN or Nursing Diploma required; BSN preferred.
+
** Certification:
** Case Management or Utilization Review certification is a plus.

** Pay Details:** $31.94 to $43.92 per hour
