Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator)
Location: Greenwood Center for Nursing and Rehab | Tamaqua, PA
Position: Full-Time
Greenwood Center for Nursing and Rehab is seeking an experienced and detail-oriented Registered Nurse Assessment Coordinator (RNAC) to join our skilled nursing facility in Tamaqua, PA. This role is critical to ensuring accurate resident assessments, regulatory compliance, and interdisciplinary coordination in alignment with state requirements and facility standards.
Position SummaryThe RN Assessment Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) and associated documentation to ensure accurate resident assessments, care planning, and compliance with federal and state regulations.
Key Responsibilities- Conduct routine and as-needed resident assessments, including condition changes and post-hospital returns
- Accurately observe, assess, and communicate resident condition changes to appropriate personnel
- Ensure timely completion of assessments and CAA documentation by interdisciplinary team members (Social Services, Activities, Dietary, etc.)
- Complete MDS sections, quarterly assessments, CAAs, and care plans when other personnel are unavailable
- Initiate and update individualized care plans based on Resident Assessment Protocols (RAPs)
- Prepare and distribute the MDS schedule to the interdisciplinary team
- Transmit MDS assessments to the state weekly (or as required) and maintain accurate records and transmittal logs
- Maintain compliance with all state and federal regulations related to resident assessments
- Current Registered Nurse (RN) license in the state of Pennsylvania
- Minimum one (1) year of experience as an MDS Assessment Coordinator in a long-term care setting
- Strong knowledge of MDS processes, care planning, and regulatory requirements
- Excellent organizational, communication, and documentation skills
