Compensation: $225,000 – $250,000 annually (based on experience)
Position OverviewWe are seeking a compassionate and clinically strong Physician (Family Medicine or Internal Medicine) to provide medical care across multiple Skilled Nursing Facilities (SNFs) in Port Charlotte. This is an excellent opportunity for a physician who enjoys longitudinal patient care, team-based practice, and a predictable weekday schedule.
You will work alongside experienced mid-level providers who support charting and lower-acuity visits, allowing you to focus on higher-complexity clinical decision‑making and patient outcomes.
Travel between facilities within a 50-mile radius is required and fully reimbursed.
Key Responsibilities- Provide comprehensive medical care to skilled nursing facility patients
- Perform evaluations, follow‑ups, and care plan oversight
- Manage chronic and acute conditions with strong clinical judgment
- Collaborate with interdisciplinary care teams to optimize patient outcomes
- Supervise and work in conjunction with mid-level providers
- Document care accurately using the PCC EMR system
- Maintain high standards of patient communication and professionalism
- Coverage across SNF in Port Charlotte
- Mid-level support for documentation and lower‑complexity visits
- No overnight shifts
- No required overtime

$225,000 – $250,000 (based on experience)
