The Occupational Therapist provides high‑quality, patient‑centered care by managing the entire process of occupational therapeutic care for an assigned group of patients. Responsibilities include assessing patients, developing and implementing individualized care plans, focusing on goal achievement and positive patient experience, and supporting patients throughout home health and the continuum of care.
Responsibilities- Treat disabilities, injuries, and disease using therapeutic techniques per physician prescriptions, perform testing and evaluation procedures on patients.
- Record and document patient observations, treatment outcomes, and changes in condition.
- Monitor, analyze, and review clinical data; modify treatment plans and goals based on patient performance.
- Communicate clearly and provide explanations of care in an accessible manner to patients and their families.
- Prioritize caseload, determine daily staffing needs, and coordinate assignments and handoffs.
- Bachelor’s Degree in Occupational Therapy;
Master’s preferred. - Experience in an appropriate healthcare setting preferred.
- Current Florida Occupational Therapist license.
- Current Basic Life Support (BLS) through AHA.
- Functional Independence Measure (FIM) required within 90 days of hire.
- Critical thinking, service excellence, good interpersonal communication skills, and PC proficiency.
- Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
- Assess and evaluate therapeutic, rehabilitative, and functional status and develop comprehensive plans of care.
- Use procedures and techniques to produce a change in condition/function consistent with physician’s plan of care.
- Observe and report patient response to treatment and changes in patient condition.
- Communicate payor needs and requirements to the treatment team, patient, and patient’s family in a timely manner.
