Social Services Specialist II

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possess a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

  Part time – 20 hours per week

  Saturday & Sunday 7:00 AM – 5:30 PM

• Preferred ACM (Case Management) or
• Preferred CCM (Case Manager)

• Required Masters in Social Work, Sociology, or Related Field for Social Services Specialist II
• Required Bachelors in Social Work, Sociology or Related Field for Social Services Specialist I

• Required 6 months Internship or 1 year Social Work in an acute care or hospital setting for Social Services Specialist II
• Required 6 months Internship or 6 months Social Work in an acute care or hospital setting for Social Services Specialist I
