Pro Med Healthcare Services is seeking a compassionate and dedicated Hospital Social Worker to join our multidisciplinary team. In this role, you will work closely with patients and their families to provide emotional support, ensure access to resources, and facilitate effective care transitions during their hospital stay.

Your responsibilities will include assessing patient needs, developing and implementing discharge plans, and advocating for patients' rights and needs within the healthcare system. You will also provide crisis intervention and support to patients facing significant health challenges, ensuring they receive comprehensive care that addresses their social, emotional, and financial needs.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to determine patient and family needs.
  • Develop and implement individualized care plans in collaboration with healthcare teams.
  • Facilitate communication between patients, families, and healthcare providers.
  • Provide crisis intervention and support, addressing immediate emotional and social needs.
  • Assist with discharge planning and connect patients to community resources and services.
  • Advocate for patients’ rights and access to necessary healthcare services.
  • Maintain accurate and timely documentation of interactions and interventions.
Requirements and Qualifications
  • Master's degree in Social Work (MSW) from an accredited program.
  • Active state licensure as a Clinical Social Worker (LCSW) preferred.
  • Previous experience in a hospital or healthcare setting is highly desirable.
  • Strong assessment, communication, and interpersonal skills.
  • Ability to work effectively within a multidisciplinary team.
  • Knowledge of community resources and services available for patients.
