Key Responsibilities Develops complex discharge plans based on members' needs assessments to ensure appropriate care levels Coordinates with providers and community agencies for safe and effective transition plans Documents and maintains discharge plans in health management systems according to policies and guidelines
Required Qualifications Graduate from an Accredited School of Nursing or Bachelor's degree in Nursing 2 - 4 years of related experience, with 2+ years in acute care Clinical knowledge of discharge planning competencies preferred Knowledge of Medicare and Medicaid regulations preferred LPN - Licensed Practical Nurse - State Licensure required
