This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office.

Duties listed but not limited to:

* The CLC Admission and Discharge Coordinator assists with patient planning assessments, identification of barriers, and activities required to successfully transition the patient into the CLC setting. The nurse facilitates an optimal transition from the CLC to the patient's next stage of recovery, either at home, in a rehabilitation facility or elsewhere. The nurse is an active member of an interdisciplinary team whose primary focus is on care transitions to and from the CLC.

Applies the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes. Monitors bed availability and serve as the point of contact for the outcome of screening decisions. Coordinates and collaborates with other VA and non-VA departments, community agencies, and healthcare providers to ensure patient needs are addressed. Provides peers with formal or informal constructive feedback for improvement.

Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level.

* The nurse applies the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes.

* Provides indirect nursing care to patients with complex needs as related to transfers, admissions, telephone triage, and discharges.

* Collaborate with the interdisciplinary team to validate and/or collect initial patient information such as Veteran CLC eligibility, facilitates physician to physician communication, nurse to nurse handoff, and bed availability in the CLC.

* Assists with patient planning assessments, identification of barriers, and activities required to successfully transition the patient into and out of the acute CLC setting.

* Participates in activities and strategies to sustain an evidence-based-practice culture.
