Duties listed but not limited to:
Responsible and accountable for all elements of the nursing process when providing direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact
customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.
Duties include, but are not limited to:
* Demonstrates clinical proficiency regarding the insertion and care of vascular access devices.
* Assesses patients with difficult access or need for long-term intravenous (IV) therapy for an appropriate vascular access device.
* Partners with staff nurses, providers, case managers, and other team members to ensure early assessment for vascular access device selection and implementation based on the patient's plan of care.
* Evaluates patients for, and places peripheral intravenous catheters, ultrasound-guided PIVs, midlines, and peripherally inserted central catheters (PICC lines) in current patient populations as outlined in policy.
* Patient & family education is provided appropriately to the type of therapy the patient is receiving.
* Provides care related to vascular access based on the age-related needs of the patient with consideration of their development/psychosocial needs, environmental safety, education needs, communication style, and level of understanding.
