Nurse Clinical Manager - Stuart, Florida

Activa Home Health, an Accord Care Company, is seeking an experienced RN Clinical Manager to lead clinical operations in our Stuart office. This is a high-impact leadership role for a Medicare home health professional with strong OASIS, case management, compliance, and team support experience.

This primarily in-office position includes clinical oversight, admissions coordination, staff mentorship, quality assurance, and field support. The ideal candidate is customer service‑driven, highly organized, tech‑savvy, and confident managing fast‑paced home health operations while ensuring compliance with Medicare and Florida home health regulations.

Responsibilities

Lead clinical operations and oversee staff in an in‑office setting, perform admissions coordination, provide staff mentorship, ensure quality assurance, and offer field support.

Qualifications
  • Current Florida RN license in good standing
  • Minimum 1 year Medicare home health case‑management experience
  • Strong knowledge of OASIS and home health compliance standards
  • EMR experience;
    Kinnser/Well Sky preferred
  • Excellent organizational, communication, leadership, and customer service skills
  • Ability to multitask and support both clinical and operational workflows
  • Team Player – Works well as a member of a group
  • Detail Oriented – Capable of carrying out a given task with all details necessary to get the task done well
