Job Summary

Primary duties of the Case Manager – Nurse (RN) will include rounding patients in the hospital, communicating with patients at bedside and other treating providers for care coordination, identifying post discharge needs and barriers to care and providing resources for patients. The Case Manager – Nurse (RN) will conduct and document assessments of patient needs and resources for effective post‑acute care.

The assessments take place throughout the hospital including inpatient, emergency room and observation units.

Role and Responsibilities
  • Regularly utilize science knowledge obtained during RN program to assist in the patient’s medical condition(s) treatment plan.
  • Maintain documentation standards in required systems (EHR).
  • Conduct thorough assessments of patients’ health status, including physical, psychosocial, and functional needs based on bedside interview with patient, family, and review of medical records.
  • Know and maintain professional communication skills: verbal, written, and non‑verbal communication.
  • Understand and maintain the highest level of HIPAA (Health Insurance Portability and Accountability) standards.
  • Qualifications
  • Education:

    Successful completion of a Registered Nurse program from an accredited college, BSN preferred; or a combination of education and relevant experience.
  • Experience:

    Minimum 2‑year practicing RN Case Manager preferred.
  • Certification/Licensure:
    Active and Clear RN License in the state of Florida. CPR/AED/BLS certified at time of hire or within 30 days.
  • Training and Knowledge Required at Time of Hire:
    Registered Nurse program curriculum completion.
