Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval – award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Former EDRP participants are ineligible to apply.

The James A… Haley VA Medical Center Geriatrics and Extended Care Service is seeking an experienced and dynamic Physician Assistant (PA) or Nurse Practitioner (NP).

• Home Based Primary Care (HBPC) provides in-home primary care services and conducts comprehensive evaluation and management of enrolled veterans including review of medical records, patient/caregiver interviews, completion of comprehensive geriatric assessment, and creation and implementation of treatment plans of care in collaboration with an attending physician.
• HBPC Program currently serves veterans living in Pasco, Polk, Hernando, Hillsborough, Citrus, and parts of Sumter County.
• Incumbent may rotate between counties to meet the needs of the patients served.
• The HBPC provider is a core member of the HBPC interdisciplinary team who participates in care management
• The provider collaborates with an HBPC physician.
• Major duties and responsibilities include: assumes the primary medical responsibility for assigned patients.
• Delivers care in the home of Veterans.
• Determines the full assessment of care for the needs of patients and caregivers.
• Identifies a patient's medical problems and defines the medical management.
• Prescribes medications and treatment in accordance with scope of practice or delineation of privileges.
• Determines the need for consultation from subspecialties.
• Determines the need for and facilitates admission to the VA medical facility and other transitions of care.
• Participates in the development of the individual plan of care at interdisciplinary care meetings for assigned panel of patients.
• Participates in the development and implementation of HBPC's process for continuous performance improvement.
• May need to cover in other sections of GEC service as a need arises, including but not limited to Hospice & Palliative Care (HPCU), Community Living Center (CLC), and GeriPACT outpatient clinic.
