Home Health Licensed Practical Nurse (LPN) – Tampa, FL
A leading healthcare organization is seeking a compassionate and dedicated Home Health Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in the home setting. Responsibilities- Provide hands-on nursing care to patients in their homes using the nursing process and established clinical standards and protocols
- Deliver patient care as ordered, including IV infusions, infusion pump management, and tube feedings under the supervision of a Registered Nurse
- Implement and maintain patient and caregiver education plans under the direction of a Registered Nurse
- Monitor patient conditions and communicate treatment responses and changes in status to the supervising Registered Nurse
- Administer medications and evaluate patient responses when appropriate
- Collaborate with interdisciplinary team members to ensure continuity and quality of care
- Maintain accurate and timely documentation of patient assessments and care provided
- Promote patient safety, independence, and positive health outcomes in the home environment
- Graduate of an accredited Practical Nursing program
- Current Licensed Practical Nurse (LPN) license in the state of practice
- Current Basic Life Support (BLS) certification
- Valid Florida driver's license or ability to obtain one within 30 days of hire
- Associate Degree or Diploma in Nursing required
- Minimum of one year of Licensed Practical Nurse experience required
- Previous home health or home care experience preferred
- Strong communication, organizational, and patient education skills
- Ability to work independently and effectively manage patient visits
- Must meet all applicable background screening requirements
