Join Pinnacle Home Care as an Associate Licensed Practical Nurse Home Health, where you will provide high-quality care to patients in their homes, ensuring comfort and well‑being while adhering to healthcare standards.
Responsibilities- Administer medications and treatments as prescribed by healthcare providers.
- Monitor patients' vital signs and report any significant changes to the healthcare team.
- Assist patients with daily living activities, such as bathing and dressing.
- Document patient care activities and maintain accurate medical records.
- Collaborate with registered nurses and healthcare team to develop patient care plans.
- Educate patients and their families on health maintenance and disease prevention.
- Ensure a safe and clean environment for patients.
- Valid state of Florida LPN license.
- Minimum of 1 year of experience as an LPN.
- Strong interpersonal and communication skills.
- Basic computer proficiency for electronic health records.
- Ability to work independently and manage time effectively.
- IV Certification
- BLS certification.
- Reliable transportation for patient visits.
