Job Overview

Licensed Practical Nurse (LPN) needed to provide one‑on‑one care to pediatric and/or adult patients with complex medical needs in the comfort of their homes. The role supports a personalized, low‑stress environment and fosters long‑term patient relationships. Responsibilities
  • Furnish services, treatments, and diagnostic and preventive procedures as assigned per the plan of care, in accordance with agency policies.
  • Prepare clinical and progress notes.
  • Assist the physician, intermediate care provider, and RN in performing specialized procedures.
  • Prepare equipment and materials for treatments, observing aseptic technique as required.
  • Assist the patient in learning appropriate self‑care techniques.
  • Understand and adhere to established policies and procedures.
  • Follow/implement the nursing care plan for each patient as approved by the RN/physician.
  • Initiate preventive and rehabilitative nursing procedures as appropriate.
  • Observe signs and symptoms and report to the physician and RN nurse reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
  • Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
  • Evaluate with RN the effectiveness of the LPN’s nursing service to the patient and family under the guidance of the RN.
  • Maintain accurate and complete records of observations, treatments and care of patient.
  • Participate in medical record audit as assigned.
  • Attend staff meetings, patient care conferences and in‑services as scheduled.
  • Takes on‑call duty, nights, weekends and holidays as assigned.
  • Submits any changes in schedule to the appropriate scheduling position in their office on a daily basis or as applicable by EMR workflow.
  • Participate in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech‑language pathologist.
  • Observe aseptic technique and standard precautions as required.
  • Accept, write and execute orders; orders are cosigned by RN where applicable.
  • Ensure that the patient/family is involved in implementing the plan of care.
  • Assist in reviewing, evaluating and updating the patient plan of care as appropriate.
  • Administer medications and treatments as ordered; document appropriately.
  • Provide appropriate information regarding patient to other agencies/individuals involved in patient’s care as needed and as directed by the RN.
  • Observe confidentiality and safeguard all patient‑related information (PHI) in accordance with HIPAA requirements.
  • Complete documentation and paperwork in a timely manner as per agency policy.
  • Immediately report/document to Nursing Supervisor/RN any patient incidents/variances or complaints.
  • Demonstrate competent performance of technical skills according to established procedures before performing new skills.
  • Participate in peer review, utilization review and QAPI activities as assigned.
  • Provide effective communication to patient/family, team members and other health care professionals.
  • Understand and adhere to established state, federal and/or accreditation policies/procedures.
  • Adhere to company standards and consistently interpret and accurately perform all assigned responsibilities.
  • Maintain acceptable attendance status, per company policy.
  • Maintain a clean and neat work environment.
  • Demonstrate sound judgment, critical thinking and decision making.
  • Perform other duties as assigned.
Qualifications
  • Graduate of an approved school of practical nursing and currently licensed in the state(s) in which practicing.
  • One year of nursing experience (preferred); new graduates welcome.
  • Active and valid CPR certification; IV certification by state mandate, if required.
  • Average computer skills with competency in media communications.
  • Physical ability to perform the described duties.
