Description:

RN
- Nurse Navigator

The Nurse Navigator serves as the coordinator for the care of patients in their specific specialties. Responsible to provide support for education, referrals, coordination of care, and other assistance to the patient and their family prior, during, and following their healthcare experience. Responsible for facilitating integrated care through communication with strategic members of the interdisciplinary team, collaborate to provide staff education and works closely with patients, families and members of the health care team to optimize patient satisfaction and achieve optimal outcomes.

Coordinates efforts between physicians, healthcare administration, family, patient resources and staff while tracking the progress of the patient throughout their continuum of care. Promotes evidence-based clinical care that exemplifies the pursuit of clinical excellence while supporting hospital and system-wide quality measures. Collaborates with local, system-wide, and Clinical Institute leadership for alignment with strategic initiatives. Provides community outreach and education opportunities, as needed.

Requirements:

• Registered Nurse license
• 2 years of RN Pediatric Acute Care
• Bachelors Degree in Nursing
• Master Degree in Nursing preferred
• BLS
• Certification in Related Specialty must be obtained within the first 6 months if not current
