What You'll DoThe Registered Nurse is a qualified professional responsible for the planning, coordination, and assurance of quality care services to patients and their families.
- Provide skilled nursing services to residents in an assisted living community.
- Complete comprehensive nursing assessments and develop individualized plans of care based on each patient's needs.
- Monitor residents' health status, chronic conditions, medications, symptoms, and progress toward goals.
- Provide ongoing nursing interventions and education to help residents manage their conditions and maintain independence.
- Identify changes in a patient's condition and communicate concerns promptly to physicians, providers, family members, and appropriate members of the care team.
- Coordinate care with physicians, specialists, therapists, facility staff, caregivers, and other healthcare professionals.
- Assist with transitions of care, including communication with hospitals, rehabilitation facilities, and other healthcare providers following a patient's hospitalization or change in condition.
- Provide patient and caregiver education related to disease management, medications, treatments, fall prevention, nutrition, and overall health and wellness.
- Coordinate needed services and resources, including DME, oxygen, infusion services, and other community-based resources.
- Maintain accurate and timely clinical documentation, including assessments, progress notes, care plans, orders, and care coordination.
- Participate in interdisciplinary care planning and collaborate with the assisted living community to promote continuity and quality of care.
- Build strong relationships with residents and their families while providing compassionate, responsive, and patient-centered care.
- Participate in ongoing education and professional development to maintain clinical knowledge and provide high-quality care.
- Active Registered Nurse License required.
- Must have a minimum of 1-year Clinical Experience
- Strong communication and organizational skills
- Ability to work independently while collaborating effectively with a multidisciplinary team

- Excellent customer service and interpersonal skills

- Valid driver's license and reliable transportation
