- Coordinate patient care by developing, implementing, and evaluating individualized care plans in collaboration with the interdisciplinary team
- Conduct thorough assessments of patients' physical and emotional needs, identifying potential risks and developing interventions to mitigate them
- Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions and continuity of care
- Participate in interdisciplinary team meetings to discuss patient progress, address concerns, and develop strategies for improved outcomes
- Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes
- Collaborate with healthcare professionals to provide high-quality, patient-centered care that meets the standards of our organization
- Current LPN or RN license in the state of Florida
- Minimum 1-2 years of experience in a long-term care or rehabilitation setting
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment with multiple priorities and deadlines
- Basic computer skills and familiarity with electronic medical records
