Job Functions:

The Case Manager provides in-office and mobile case management services; evaluates and assesses immediate needs; coaches clients in the development of self-sufficiency plans; functions as a liaison between clients, community service providers and program component staff. In addition, the Case Manager connects families with services and refers First Hug and other services for families.

Essential Responsibilities:

  • Provide Case Management services to Emergency Shelter families, developing a treatment plan (Case plan), coordinating all services for families, and directing clients to other areas of assistance.
  • Participate in weekly staff and group meetings. Organize and lead meetings for family related decision making.
  • Maintains client case notes, history, and data, utilizing best practices to keep client information confidential according to the Code of Ethics.
  • Maintain accurate client information at program entry as well as at exit to meet the contractual expectations of the Emergency Shelter Program.
  • Maintain professional boundaries.
  • Coordinate with all Metropolitan Ministries programs staff to ensure each service will be utilized to best meet the needs of the family. Incorporate all staff in follow-through of service utilization.
  • Work closely with community partners to develop plans for assisting with specific family needs. Provide training opportunities for staff and community providers on Emergency Shelter services.
  • Maintain minimum weekly contact with Families to track program progress.
  • Develop and implement group learning opportunities for Families and their children.
  • Respond to emergency situations related to assigned families and provide any information or services to support overnight team members.
  • Maintain all Emergency Shelter forms and documents needed for working with Emergency Shelter families.
  • Visit and meet with community service providers, service organizations and institutions which prove beneficial to the maintenance of self-sufficiency.
  • Assist with client room inspections when assistance is needed from the CLA team.
  • Meet personal goals and actively contributes to the achievement of departmental and organizational goals.
  • Support team in other tasks as assigned. Attend all required staff meetings and on-going trainings. Support the holiday tent operations as directed.
  • Requirements:

    Education and Experience:

    A minimum of a Bachelor’s degree in the human services field and two years of work experience. Equivalent combination of education and experience will be considered. Must be knowledgeable about homeless issues, with experience working with individuals with mental health/substance abuse issues and/or be willing to seek continuing education related to the homeless population with opportunities for certification or licensure.
