Summary of the Position
:
Case Manager provides intensive individual mental health interventions and support services. Case Manager is supervised by a mental health professional as part of the treatment team.
- Assist in the development and implement the interventions developed by the Treatment Teams and describe them in the treatment plan.
- Provide clinical information and data to mental health professionals and other pertinent professionals to support the treatment plan’s objectives, engage participants in age-appropriate therapeutic activities, and provide information and data in a timely manner.
- Participate in and meet any training or testing requirements that may be required by the state government and/or referring authority.
- Master's degree in psychology, sociology, social work, or related field preferred
- Bachelor's degree in psychology, sociology, social work, or related field required with at least one year of experience in the CASSP system program or 12 college-level semester hours in social services or humanities with at least one year of experience in the CASSP system program
- Bachelor's degree or 6 years working with youth and their families
- Experience in a community-based agency program
- Experience working with youth and families in a behavioral health context and/or at-risk youth and families
- Strong written and verbal communication skills
- Proficient in Microsoft Office Suite; experience using an electronic health record (EHR) system is a plus.
- Position requires reliable transportation, valid driver's license, and current auto insurance coverage.
- Bilingual (Spanish) is a plus
