**** Position Overview
** Summary Under the general supervision of the Clinical Supervisor or designee, this position works to make a meaningful difference in the lives of the children and families served. This position works closely with youth, family, foster parents, and community partners, providing advocacy and support toward the ultimate goal of living safely and successfully in the community.
** Responsibilities
* ** Actively supports, represents, and extends the mission, vision, and values of the organization.
* Provides case management, transportation, and other professional services, meeting or exceeding established outcomes thresholds on behalf of persons served.
* Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.
* Completes all required documentation in a timely manner.
* Completes all required training in a timely manner.
* Coordinate and monitor services for the youth and family in the community, including but not limited to, mental health, medical, educational, psychological, vocational, and social services.
* Regularly reviews and modifies youth based plans to assure consistent progress and success on written plan goals.
* Establishes and maintains strong relationship with assigned foster parents and families of origin.
* Assists in family reunification and/or stabilization efforts with the youth’s primary family or other permanent living arrangement consistent with the youth’s case plan.
* Provides safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served.
* Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis.
* Actively participates in the weekly supervision process.
* Conducts public relations activities in the communites where NYAP resides on behalf of the foster care program through the media, publicatons and other iinitiatives outlined in the state/regional recruitment plan.
* Works to produce flyers, advertising, and inform atonal brochures for use in public relatons activities.
* Works directly with community leaders in fostering the mission of the National Youth Advocate Program.
* Works directly with potential foster parents in meeting state pre-service training requirements.
* Works directly with potential foster parents in assisting them with completing all paperwork necessary to become a foster parent with the National Youth Advocate Program regarding the regulations set forth by the appropriate regulatory agencies.
* Works directly with existing foster parents in the recertification process by helping them maintain their eligibility to serve children as a foster parent with the National Youth Advocate Program.
* Serve as a resource coordinator in helping foster parents seek all training opportunities available to them through the National Youth Advocate Program and in the community.
