Transition Service Coordinator

Transitional Service Coordinator – Tampa Bay Girls Academy – Tampa, Florida

Rite of Passage Team is hiring a Transitional Service Coordinator to supervise a caseload of approximately 14‑24 clients, including DOC clients and ROP Aftercare students, in a residential program serving high‑risk and max‑risk youths aged 12‑21.

• Supervise a caseload of 14‑24 clients, ensuring program participation and providing transportation as needed.
• Document all contacts and submit weekly reports to the Transitional Services Manager.
• Identify and coordinate with community contacts and other program staff to track and monitor client progress.
• Coordinate activities, establish contacts, and identify opportunities for client employment.

• BA/BS in a related field preferred.
• At least one year of experience working with at‑risk juveniles and their families, or comparable experience.
• Must meet the requirements to be an eligible ROP driver and possess a current State Driver’s License with an acceptable driving record for the past three years.
• Strong knowledge of overall company operations, policies, and procedures.
• Must pass a critical background clearance check, drug screen, medical physical, and TB test.
