COMMUNITY OUTREACH SPECIALIST
Position Summary:
This is a responsible professional position of Community Affairs which involves contact with the community and customers for the Agency for Persons with Disability in an effort to fill unmet needs for the Agency’s customers.
The Agency seeks team members who demonstrate the qualities of being humble, hungry, and smart and lead others to embody the same. This entails lacking excessive ego, sharing credit, being self‑motivated, and exercising good judgment and intuition regarding the subtleties of group dynamics.
This position with APD will serve staff and management across the Agency. Your work will impact the workplace culture, our community, and the population we serve as we continuously strive to achieve the Agency’s mission.
Work You Will Do- Family Care Council Support: Serve as the primary liaison to the Family Care Councils in SCR. Facilitate collaboration between regional FCC and newly developed FCC State. Coordinate council meetings and ensure follow‑up on action items. Facilitate communication between council members and agency leadership. Promote council initiatives and assist in recruiting new members.
- Community Events and Outreach: Plan, organize, and represent the agency at community outreach events and resource fairs. Build partnerships with community organizations, stakeholders, and advocacy groups. Develop and distribute outreach materials to educate the public about available services. Monitor outreach effectiveness and provide recommendations for improvement.
- Medicaid Technical Assistance: Provide technical assistance and support to individuals and families navigating Medicaid eligibility determinations. Assist with eligibility inquiries, application processes, and service coordination. Collaborate with Medicaid providers to address systemic issues and improve service delivery. Maintain up‑to‑date knowledge of Medicaid policies and ensure compliance with state and federal guidelines.
- Emergency Management Support: Participate in emergency preparedness planning and training with a focus on Independent Living/Supported Living settings. Coordinate with local agencies to ensure continuity of services during emergencies. Assist with response and recovery efforts as needed, including communication and logistical support.
- Project Transition Council: Support the Project Transition Council meetings held in each county focused on youth transitioning from school to adult life. Facilitate interagency collaboration and ensure follow‑up on identified needs and strategies. Document and track progress of transition‑related goals and initiatives.
- Other Duties: Perform additional tasks assigned to support the agency’s mission and programs. Contribute to special projects, reports, and internal improvement initiatives.
- High school Diploma or its equivalent.
- Must have valid Driver’s License or other efficient means of transportation to travel for work purposes.
- Must have knowledge and at least two years professional work experience with Developmental Disabilities or Mental Illness, or the principles and practices of one of the allied health field principles or one of the rehabilitative sciences.
Knowledge of:
- Using databases, electronic files, and Microsoft Office Programs.
