GS-09 Social Worker

Duties

Duties include, but are not limited to:

  • The GS-09 Social Worker carries out duties under the supervision of a Licensed Clinical Social Worker (LCSW) who is a qualified supervisor. This supervisor may not necessarily be from the same area where the incumbent works. Additionally, incumbents may receive day-to-day oversight from their section supervisor as needed.
  • The Social Worker reports directly to the Non-Institutional Care Supervisor for Social Work Service. In general, the Social Worker practices independently, consistently exercising independent judgment in day-to-day activities. Supervision/consultation is of an administrative and professional nature; with the Supervisory Social Worker, providing guidelines to the general policy and direction of the Non-Institutional Care section.
  • The OCC Placement Specialist reports directly to the assigned PACT Supervisor for Social Work Service. In general, the Social Worker practices independently, consistently exercising independent judgment in day-to-day activities. Supervision/consultation is of an administrative and professional nature; with the Supervisory Social Worker, providing guidelines to the general policy and direction of the Non-Institutional Care section.
  • The social worker is also responsible to the Chief of Social Work Service for professional practice issues. The social worker is expected to participate in professional activities within his/her respective discipline and, when needed, consults with the Chief of Social Work regarding discipline specific concerns.
  • The Social Worker is responsible for independently discharging the mandates of the position. It is the incumbent's responsibility to solicit consultation as appropriate. The incumbents' ongoing performance will be reviewed through direct supervision, reports, review of work, and information from others such as VA facility leadership and department leaders.
  • Care provided may include assessments and case management of:
    • Placement of Veterans from non-VA medical facilities to the next level of care.
    • Responsible for assisting Office of Integrated Veteran Care (OIVC) patients with VA contract referral requests.
    • May be responsible for assisting with other patient placements as determined by assigned supervisor.
  • Independently performs case finding functions and outreach, as well as identify and screen patients for psychosocial needs. Implements treatment modalities, provide counseling or psychotherapy for individuals, families and groups.
  • Conducts psychosocial assessments, develop treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team, and offers psychosocial interventions and measures outcomes of interventions.
  • Coordinates community-based services, including information and referral for those services including VA programs, other government programs and community agency programs.
  • Makes rapid assessments and developing crisis management plans for admission to acute, short term, and/or long term placements. Utilizes multiple resource directories in addition to previously developed resources to meet the demands of a crisis.
  • Independently identifies high-risk patients and provides case management services. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
  • Establishes and maintains effective therapeutic relationships with Veterans/patients and their families and/or caregivers. Works with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group and family counseling and therapy skills.
  • Maintains general knowledge of Veterans' benefits and services relating to special programs, service connected compensation and non-service connected pension. Maintains advanced knowledge of community resources as well as VA programs that support the psychosocial and financial needs of Veterans living independently in the community and facilitates referrals.
Work Schedule

The primary tour of duty will be M-F 7:30am to 4:00pm.
