Independent Living Transition Facilitator

This position requires trained professionals to connect with 20 to 30 youth ages 16 and 17 in licensed foster care settings to yield a productive transition plan. Facilitators meet young people “where they are” and build a trusting relationship with them using an authentic, trauma-informed, strength-based approach. A designated child welfare professional that establishes a positive relationship with the Transitioning Youth, proficient in both case management and independent living services, that takes the lead in coordinating the development of the “My Pathway to Success” transition plan and ensures transition activities are satisfied for each youth.

The Transition Facilitator takes the lead role in ensuring that each Transitioning Youth has an opportunity to develop a comprehensive plan and receives the required records and resources necessary to help him or her become self-sufficient.

A Bachelor’s degree in a Human Service discipline and 2+ years of experience working with young people with diverse needs and attributes.
