HEALTH CONNECT AMERICA, INC is seeking a Bilingual Targeted Case Manager to support children, adults, and families with behavioral health needs. In this role, you will assess client needs, develop service plans, coordinate referrals, and advocate for clients across community, in home, and office settings. You will collaborate with therapists, clinicians, and community partners to ensure comprehensive, culturally responsive care. Responsibilities include monitoring progress, maintaining accurate documentation, and linking clients to social, educational, and healthcare resources while supporting recovery and stability in their local communities.
Responsibilities
- Conduct biopsychosocial and needs assessments for clients and families
- Develop, implement, and regularly update individualized service and care plans
- Coordinate referrals to behavioral health, medical, educational, and social services
- Provide bilingual communication and support to clients and families
- Advocate for clients within schools, courts, and community agencies
- Monitor client progress, outcomes, and service effectiveness
- Maintain timely, accurate, and compliant documentation and case records
- Collaborate with therapists, clinicians, and community partners in multidisciplinary teams
- Identify and connect clients to community resources to address housing, employment, and basic needs
- Participate in trainings, supervision, and quality improvement activities
