Child Welfare Case Manager

Lutheran Services Florida is seeking a Child Welfare Case Manager to support children and families involved with the child welfare system. This role conducts home visits, assessments, and safety planning; coordinates services such as healthcare, counseling, and education; and advocates in court and with community partners. The Case Manager maintains accurate documentation, develops family-centered case plans, and monitors progress toward reunification or permanency.

Working within LSF's mission-driven, faith-informed culture, you will collaborate with a multidisciplinary team to protect children, strengthen families, and connect them with vital community resources and supports.

Responsibilities

• Conduct home visits and comprehensive assessments of child and family safety, risk, and needs.
• Develop and implement individualized case plans focused on safety, stability, and permanency.
• Coordinate and monitor services such as healthcare, counseling, education, and housing support.
• Prepare and maintain accurate, timely case notes, reports, and legal documentation.
• Advocate for children and families in court, school, and with community partners.
• Collaborate with supervisors, clinicians, and community agencies to support case goals.
• Respond to crises and implement safety plans to protect children from harm.
• Engage families using strengths-based, culturally competent, and trauma-informed practices.
