Lutheran Services Florida is seeking a Family Services Case Manager to support children and families through assessment, service planning, and ongoing case coordination. In this role, you will manage a caseload, complete home visits, connect clients with behavioral health, education, and community resources, and advocate for safety, stability, and well-being. You will collaborate with multidisciplinary teams, document services in compliance with agency and state standards, and participate in case staffings and court-related activities as needed.

Responsibilities

  • Manage a caseload of children and families, providing comprehensive case management services
  • Conduct intake and ongoing psychosocial assessments and develop individualized service plans
  • Perform regular home and community visits to monitor safety, progress, and family needs
  • Coordinate referrals to behavioral health, education, medical, and community resources
  • Advocate for client safety, stability, and access to needed services within child- and family-serving systems
  • Maintain accurate, timely documentation in accordance with agency, state, and funding requirements
  • Participate in multidisciplinary team meetings, staffings, and court-related activities as required
  • Educate families on available services, rights, and responsibilities to support long-term stability
  • Respond to crises and safety concerns using appropriate de-escalation and reporting procedures
  • Collaborate with supervisors and colleagues to improve service quality and client outcomes
