The OpportunityWe are currently seeking a dedicated Social Services Director
.
- Assist in the planning, organization, implementation, and evaluation of the facility’s Social Services Department
- Ensure social services are delivered in compliance with federal, state, and local guidelines and in alignment with Harborview's values and policies
- Help develop and monitor individualized care plans for residents, addressing their social and emotional needs
- Provide ongoing support to residents and families during admissions, transitions, and care conferences
- Recommend appropriate equipment, services, or referrals for outpatient and home health support
- Participate in policy development and recommend improvements to strengthen care delivery
- Organize and participate in in-service training sessions for the social services team and other staff
- Stay up-to-date on regulatory changes and professional standards related to long-term care social work
- Advocate for residents and serve as a liaison among staff, families, and external agencies
- Collaborate with interdisciplinary care teams to ensure holistic, person-centered care
- Duties as assigned
- Bachelor’s degree in Social Work (BSW), or in a human services field such as sociology, psychology, special education, or rehabilitation counseling
- Minimum of 1 year supervised social work experience in a healthcare setting working directly with individuals
- Knowledge of federal and state long‑term care regulations
- Strong interpersonal, communication, and problem‑solving skills
- A resident‑first mindset with a deep commitment to dignity, empathy, and respect
- Ability to manage sensitive situations with professionalism and compassion
- Must meet all applicable licensure and certification requirements in the state of practice
