Licensed Practical Nurse I – Day Shift (260004YG)

Taunton State Hospital is seeking a Licensed Practical Nurse I to provide nursing care to patients in an assigned area; to provide daily care and personal or hygiene needs; to observe and record patient behavior; and to perform related work as required.

Shift: 6:45 a.m. – 3:15 p.m., 40 hours per week on the Cain 3 West unit.
Days off: Week 1 – Sunday/Friday;
Week 2 – Wednesday/Saturday. Employees will be required to perform mandatory overtime.

• Provides direct nursing care to patients – taking and charting vital signs, applying and changing medical and surgical dressings, administering medications, monitoring fluid intake and output, performing basic first aid.
• Provides for daily care and personal/hygiene needs – baths, feedings, grooming, skincare, enemas, etc.
• Supports patient independence in activities of daily living, supervises meals, escorts to and supervises group activities, and orientates patients to the unit.
• Treats all patients with dignity and respect, advocates for patients, facilitates timely resolution of concerns, and assists patients in exercising their rights.
• Develops therapeutic relationships with patients, families, and the care team.
• Participates in maintaining a therapeutic milieu, attending rounds, and collaborating in data collection for assessment, planning, and evaluation.
• Contributes to nursing care plans and multidisciplinary treatment plans.
• Responds to medical and psychiatric emergencies and ensures emergency equipment is available and inspected.
• Provides health education to patients and families per nursing care and treatment plans.
• Administers treatments and medications per hospital policy.
• Documents patient care in the medical record and transcribes MD orders.
• Completes mandatory trainings annually and participates in continuing education as required by the Board of Registration.
• Performs related duties such as maintaining records, retrieving and transporting equipment, supplies, and laboratory specimens, transferring patients, and scheduling appointments.
• Performs other related duties as assigned.

• Practical nursing knowledge and skills; patient handling and transportation techniques.
• Proficiency in English grammar, spelling, and terminology for nursing practice.
• First aid techniques and custodial care for special client groups.
• Knowledge of standard nursing methods, medical and surgical dressings, emergency procedures (CPR, tourniquets).
• Blood and urine sample collection skills.
• Understanding of nursing practice for geriatrics and human anatomy.
• Ability to follow written and verbal instructions; to observe and gather information; to communicate effectively.
• Teamwork, cultural competence, judgment, accurate record‑keeping, and the ability to remain calm under stress.
• Physical capability to lift heavy objects or patients.

• Bilingual or multilingual fluency given the patient population.

Taunton State Hospital (TSH), operated by the Department of Mental Health (DMH), is a 62‑bed inpatient facility providing continuing care for adults with major mental illnesses. TSH emphasizes recovery and rehabilitation philosophies and collaborates closely with outpatient teams to facilitate community discharge.
