Summary

The Physician Assistant (PA) or Nurse Practitioner (NP) provides direct detainee care within the Immigration and Customs Enforcement Health Service Corps. Responsibilities include clinical evaluation, minor procedures and medication prescriptions, reporting to the Supervisory APP or facility Clinical Director, and maintaining compliance with IHSC policies.

Duties and Responsibilities O‑3 & O‑4 Billet
  • Practice in accordance with established criteria, guidelines and agency procedures.
  • Perform comprehensive history and physical evaluations, behavioral health care, and minor outpatient surgical procedures.
  • Coordinate health‑care needs of detainees, including referrals to off‑site consultants.
  • Take infection‑control actions based on IHSC policy.
  • Complete required training and attend on‑site, regional, and national meetings.
  • Participate in the APP Peer Review process at O‑4 level.
  • Perform other duties as assigned.
O‑5 Billet
  • Provide direct care through scheduled appointments, emergency or urgent visits.
  • Consult with the physician/Clinical Director for complex cases.
  • Manage infirmary care including admission, discharge, and daily rounds.
  • Conduct comprehensive evaluations and minor surgical procedures.
  • Apply drug and therapeutic knowledge when prescribing medication.
  • Interpret diagnostic and laboratory results, including TSTs and TB management (Use the "Apply for this Job" box below). to universal precautions and properly dispose of sharps.
  • Serve as medical escort for detainees as needed.
  • Provide interpretation, translation or assisted communication services for LEP or disabled detainees.
  • Document patient care in required health record format.
  • Serve as Acting Senior Supervisory APP or Acting Regional APP.
  • Mentor APP onboarding and training.
  • Participate in APP Peer Review process.
  • Perform other duties as assigned.
Requirements
  • U.S. citizen.
  • Pass background investigation and drug screen.
  • Male applicants born after 12/31/59 must register with Selective Service.
  • Service commitment of 18–24 months upon appointment.
  • No current Assignment Pay Agreement.
  • Currently a USPHS Commissioned Corps Officer or USPHS Call to Active Duty (CAD) candidate.

Physical Demands
  • Possible lifting of >30 lbs and performing CPR/emergency care.
  • Assist sick, injured or aging detainees which may require lifting or carrying > self‑weight.
  • Run short distances, prolonged standing and work in austere environments.
Education and Knowledge, Skills, and Abilities O‑3 & O‑4 Billet
  • New graduate or no clinical experience for O‑3; or 1 year of clinical experience for O‑4.
  • Meeting minimum education and certification requirements for PA or NP.
  • Unrestricted state license and national certification for PA or NP.
  • DEA unrestricted license to prescribe controlled medications.
  • Current CPR/BLS certification and readiness for duty.
  • Preferred: experience in correctional health care, IHSC policy familiarity, TB management, ACLS, DEA Buprenorphine X Waiver.
O‑5 Billet
  • More than 4 years of full‑time clinical experience or approval by IHSC Medical Director.
  • Team‑lead or supervisory experience.
  • Unrestricted PA or NP license with national certification.
  • Current CPR/BLS certification and readiness for duty.
  • Preferred: correctional health care experience, CCHP certification, IHSC policy knowledge, TB management, ACLS.
