We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients’ homes, under supervision of a Registered Nurse, ensuring adherence to care plans, physician orders, and confidentiality standards.
Key responsibilities include:
- Delivering quality patient care and coordinating services according to physician instructions
- Monitoring and assessing patient conditions, reporting changes promptly
- Documenting assessments, actions, and responses accurately and timely
- Implementing and updating nursing care plans, taking appropriate actions based on assessments
- Recognizing and addressing patient needs to prevent risks
- Educating patients and families on disease management, self-care, and prevention
- Maintaining infection control and complying with regulations and policies
Qualifications and skills include:
- Active, unencumbered LPN/LVN license in the practicing state
- Current CPR certification
- TB test within last 12 months
- Reliable transportation
- Preferred: six months of clinical experience and home health background
- Excellent attention to detail, time management, communication, and problem-solving skills
Physical requirements: ability to lift 50 pounds, reposition patients, travel, and respond to emergency situations. The role may involve exposure to blood, bodily fluids, and infectious diseases. The environment varies and may include allergens and other conditions.
