Health RIGHT 360 offers residential treatment services designed to serve individuals and families with substance use disorders and co-occurring mental health disorders. Our program helps clients address addiction and related issues, increase skills, manage stressors, connect to supportive service, and build a foundation for lasting recovery. Residential programs run 24/7 and include individual and group therapy, family therapy, individualized case management, treatment planning, drug and alcohol counseling, medication support services, and crisis intervention.

JOB SUMMARY

Health RIGHT 360, a family of programs, operates in 11 counties throughout California. Offering a comprehensive range of medical and behavioral health services, Health RIGHT 360 serves marginalized communities, including primary care, mental health support, detoxification, residential and outpatient substance use disorder treatment, and transitional services tailored to individuals involved in the criminal justice system.

North County Serenity House (NCSH), a residential program of Health RIGHT 360, was founded in 1966 to provide substance use disorder services in the community. NCSH provides a gender-responsive and trauma-informed environment, using evidence-based and best practices that recognize and account for the role that trauma frequently plays in substance use and criminal histories of women. For clients with co-occurring mental illness, we provide integrated substance use and mental health services which treat both conditions as primary.

Our residential facility serves up to 120 women (with capacity for up to 20 children under 5 years of age) seeking recovery from substance use disorders.

The Care Coordinator II, Special Projects (SP) provides comprehensive support and advocacy for our clients y assess the needs and strengths of our clients and their children to promote family health and recovery. They provide case management in both individual and group settings, deliver program-specific curricula, and assist clients with children in navigating systems of care within a supportive environment. Care Coordinator II, SP acts as a liaison between the program and Child Welfare Services to ensure comprehensive care for our clients.

KEY RESPONSIBILITIES

Direct Client Interaction and Support:

• Facilitate individual sessions with each caseload client who is involved with Child Welfare Services (CWS).
• Provide learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals.
• Perform crisis intervention and communicate with the treatment team as unforeseen situations arise.
• Facilitate groups, assessments, and treatment plans.
• Collaborate with each caseload client and other available internal and external resources to develop and maintain treatment plans, transition plans, progress notes, and appropriate updates in support of the health and recovery needs of the client.
• Proactively link clients to both internal and external resources based on their treatment needs and follows up on the progress/status.
