Responsibilities
- Direct Patient Care:
- Triage patients’ concerns to determine appropriate care needed.
- Perform clinical procedures such as injections, wound care, dressing changes, catheter insertion/removal, and administering breathing treatments.
- Respond to lifesaving situations based upon nursing standards, policies, procedures, and protocols.
- Clinical Administration:
- Respond to patient telephone inquiries and provider requests regarding medical concerns, lab results, refills, and referrals.
- Provide provider coverage by determining the urgency of pending lab results, diagnostic imaging, and medication refills requested.
- Document patient encounters and care plans in the electronic health records system.
- Provide clinical coaching support and indirect supervision to medical assistants.
- Complete ongoing/annual training for position compliance.
- Provide shift coverage at other Neighborhood sites as directed.
- Immunization Program Support/Oversight:
- Review and compare needed records and abstract data to California Immunization Registry (CAIR), eClinical
Works (eCW), and patients’ immunization records, including identifying when immunizations are due. - Verify immunization orders for accuracy, such as correct vaccine, dosage, expiration date, and age.
- Provide vaccine information statements (VIS) and administer vaccines as ordered.
- Document vaccines administered in CAIR, eCW, and patient vaccine records.
- Schedule recall appointments for upcoming due vaccines.
- Ensure all forms are kept up to date, such as VIS forms.
- Complete and submit required reports, including vaccines administered, reminder/recall reports, state flu reports, and other reports as necessary.
- Manage vaccine inventory to maintain accurate inventory, manage supply expiration dates, and rotate stock to avoid waste.
- Place vaccine orders in a timely manner to prevent running out of vaccines or creating an oversupply.
- Perform routine cleaning standards to maintain refrigerators and freezers for vaccines.
- Conduct proper handling, storage, and management of vaccines in compliance with Vaccines for Children (VFC) / Vaccines for Adult (VFA) Guidelines.
- Provide staff immunization training in conjunction with medical assistant training as needed.
- Provide periodic vaccine updates, in-services, or information to staff as needed.
- Monitor and record refrigerator and freezer temperatures at least twice a day.
- Assist vaccine excursions in alignment to standard protocols.

- Patient
Education:- Initiate a patient education plan and provide education according to individual needs as prescribed by provider and/or clinic policy, including patient and family instructions.
- Provide ongoing support and continuous assessment for international normalized ratio (INR) patients.
- Educate and instruct patients regarding lab tests, results, medication functions, and expectations to gain cooperation and allay apprehension.
- Associate’s degree in nursing required;
Bachelor’s degree in nursing preferred. - CA RN license required.
- Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source required upon hire and must be maintained as a condition of employment.
- Two years’ clinical, outpatient, or general medicine experience preferred.
- Bilingual (English/Spanish) preferred.
- Valid CA driver’s license or out‑of‑state driver’s license for active military personnel/spouses required.
- Proof of auto insurance required.
- Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills.
- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.
- Ability to apply critical thinking and sound judgment with confidence.
- Knowledgeable about and experienced in delivering care while exemplifying compassion to patients and team members.
- Knowledgeable about and experienced with electronic health records systems.
- Ability to apply efficient nursing techniques with confidence.
- Ability to use emotional and cultural intelligence to provide appropriate communication and resolve conflicts.
