Responsibilities

  • Direct Patient Care:
    • Triage patients’ concerns to determine appropriate care needed.
    • Perform clinical procedures such as injections, wound care, dressing changes, catheter insertion/removal, and administering breathing treatments.
    • Respond to lifesaving situations based upon nursing standards, policies, procedures, and protocols.
  • Clinical Administration:
    • Respond to patient telephone inquiries and provider requests regarding medical concerns, lab results, refills, and referrals.
    • Provide provider coverage by determining the urgency of pending lab results, diagnostic imaging, and medication refills requested.
    • Document patient encounters and care plans in the electronic health records system.
    • Provide clinical coaching support and indirect supervision to medical assistants.
    • Complete ongoing/annual training for position compliance.
    • Provide shift coverage at other Neighborhood sites as directed.
  • Immunization Program Support/Oversight:
    • Review and compare needed records and abstract data to California Immunization Registry (CAIR), eClinical

      Works (eCW), and patients’ immunization records, including identifying when immunizations are due.
    • Verify immunization orders for accuracy, such as correct vaccine, dosage, expiration date, and age.
    • Provide vaccine information statements (VIS) and administer vaccines as ordered.
    • Document vaccines administered in CAIR, eCW, and patient vaccine records.
    • Schedule recall appointments for upcoming due vaccines.
    • Ensure all forms are kept up to date, such as VIS forms.
    • Complete and submit required reports, including vaccines administered, reminder/recall reports, state flu reports, and other reports as necessary.
    • Manage vaccine inventory to maintain accurate inventory, manage supply expiration dates, and rotate stock to avoid waste.
    • Place vaccine orders in a timely manner to prevent running out of vaccines or creating an oversupply.
    • Perform routine cleaning standards to maintain refrigerators and freezers for vaccines.
    • Conduct proper handling, storage, and management of vaccines in compliance with Vaccines for Children (VFC) / Vaccines for Adult (VFA) Guidelines.
    • Provide staff immunization training in conjunction with medical assistant training as needed.
    • Provide periodic vaccine updates, in-services, or information to staff as needed.
    • Monitor and record refrigerator and freezer temperatures at least twice a day.
    • Assist vaccine excursions in alignment to standard protocols.
  • Patient

    Education:

    • Initiate a patient education plan and provide education according to individual needs as prescribed by provider and/or clinic policy, including patient and family instructions.
    • Provide ongoing support and continuous assessment for international normalized ratio (INR) patients.
    • Educate and instruct patients regarding lab tests, results, medication functions, and expectations to gain cooperation and allay apprehension.
Education / Experience
  • Associate’s degree in nursing required;
    Bachelor’s degree in nursing preferred.
  • CA RN license required.
  • Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source required upon hire and must be maintained as a condition of employment.
  • Two years’ clinical, outpatient, or general medicine experience preferred.
  • Bilingual (English/Spanish) preferred.
  • Valid CA driver’s license or out‑of‑state driver’s license for active military personnel/spouses required.
  • Proof of auto insurance required.
