We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills.

Your Role in our Company Will Be:
The RN travels to a patient's home to administer their services and helps patients maintain their independence. Their duties include:
- Administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans.
- Updating Doctors & The Agency about their patient's health.
- Documentation and communication with the Agency is done through Synergy Software.
AVAILABILITY:
Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).
Work Area:
City close to you, with a group of patients who need care for that day.
RESPONSIBILITIES
- Providing nurse visits to home health patients.
- Performing an initial, comprehensive assessment; documenting and uploading to Agency Portal.
- Obtaining and documenting physician orders.
- In consultation with the assigned Clinical Team Manager, initiating and regularly re-evaluating and revising the plan of care.
- Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team; informing them of significant changes in the patient's condition and needs.
- Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager; completing and submitting all related documentation.
- Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA).
- Performing Home Health Aide supervisory visits per regulatory requirements.
- Demonstrate knowledge of regulatory requirements.
POSITION QUALIFICATIONS
- Visiting RN for Home Health: 1 year (Required)
- Nursing: 1 year (Required)
- License/Certification:
- Current CA RN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)
