This is a full‑time role for a Home Health and Hospice RN. We will train in‑house through lots of shadowing with another RN. As a Home Health RN, you will play a critical role in providing excellent patient care in the comfort of their own homes.
Responsibilities- Assessing patient needs and creating care plans
- Administering medications and monitoring vital signs
- Providing wound care
- Collaborating with a multidisciplinary team to ensure holistic well‑being and help patients achieve their health goals
- Valid RN license in the state of CA
- Minimum of 3 years of experience as a registered nurse is a plus
- Experience in home health or a related setting is preferred
- Hospice experience is a plus
- Excellent clinical assessment and critical thinking skills
- Strong communication and interpersonal skills
- Ability to work independently and manage your time effectively
- Compassionate and empathetic approach to patient care
- Proficiency in electronic medical record (EMR) systems
- Current BLS certification
- Acute care clinical experience is a plus
