Option Care Health is seeking a Clinical Transition Educator (RN) to provide clinical education and training to ensure successful transitions to our home infusion model. This role partners with the sales team to increase patient transitions and delivers in-person, virtual, and on-site education.
You will assess patient suitability, coordinate with care facilities, maintain confidentiality, and document education using OCH tools. Local travel to facilities is 100%.
