Occupational Therapist OT Home Health

Position Summary

Banner Home Health seeks a full‑time Occupational Therapist (OT) to provide therapy in patients’ homes across Mesa, Tempe, Chandler, and Phoenix. The OT will evaluate and treat patients following physician referrals, develop and implement individualized treatment plans, and collaborate with the interdisciplinary team to promote recovery and independence.

• Evaluate patient functional abilities, determine intervention needs, and establish measurable treatment goals.
• Implement individualized treatment plans that consider the patient’s physical, social, spiritual, cultural, educational, and age‑specific needs.
• Document progress and treatment techniques in accordance with department and regulatory standards.
• Communicate patient progress and treatment rationale to physicians, nurses, family caregivers, and the interdisciplinary team.
• Adapt caseload and scheduling as needed, providing assistance in other patient care areas when appropriate.
• Provide direction and delegation to COTA, therapy technicians, and students to ensure safe, cost‑effective care.
• Participate in departmental process improvements and meet productivity standards.
• Interact with all levels of staff and external contacts to support patient health.

• Successfully completed an educational program in occupational therapy approved by the governing board.
• Current Occupational Therapy license from the State of practice.
• BLS certification required.
• Ability to become proficient with commonly used computer software applications.
• Arizona Fingerprint Clearance Card at the time of hire and maintenance throughout employment.

• Additional related education and/or experience preferred.
