DES is looking for individuals who are committed to service, community, and teamwork. The Department of Economic Security, Division of Developmental Disabilities (DDD), seeks a Case Manager 2 - Targeted Unit in the Specialty District. DDD provides services and supports to individuals with developmental disabilities and their families that are flexible, high quality, and member driven. These services enable individuals to exercise their rights and responsibilities of independent decision-making and community engagement.

Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.

• Drive a vehicle to conduct onsite visits to members’ homes, schools and/or program sites to develop relationships and discuss service needs and supports.
• Develop, review, monitor and evaluate Person-Centered Service Plans (PCSP) in accordance with mandated timelines and document progress and concerns; complete appropriate referrals to community agencies.
• Facilitate meetings and program visits to assist with member/family choices; complete referrals for assessed/covered services.
• Collaborate with community agencies to provide assistance and support to members and families in crisis.
• Write comprehensive progress notes and maintain case records; prepare a variety of reports and complete special projects as required.

• Knowledge in: HIPAA; medical and behavioral health issues and terminology; methods, procedures, and techniques for PCSP development; federal and state statutes and agency policies related to care for individuals with developmental disabilities; principles and practices of case management.
• Skills In: developing, evaluating, and maintaining member service plans; interviewing, counseling, and interacting with members and families; problem identification and resolution.
• Ability to: work independently and in diverse teams; organize and manage caseload; document case files within timelines; maintain confidentiality; drive a motor vehicle.

The ideal candidate for this position will have: N/A

• Candidate must be an Arizona licensed Registered Nurse in good standing OR possess a Bachelor Degree in Social Work (SW), Psychology, Special Education or Counseling AND have at least one (1) year experience providing case management services to elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental Illness (SMI); OR have at least two (2) years’ experience providing case management services to such populations.
• A Bachelor’s Degree in Human Services, Behavioral Science, or related field AND at least one (1) year of case management experience with the specified populations OR at least three (3) years’ experience working with individuals with intellectual or developmental disabilities.
• If driving is an essential function, Driver’s License requirements: valid driver’s license, training, driving record checks, and insurance as applicable.
• Valid Level One fingerprint clearance card to work with children and vulnerable adults.
• Candidates must undergo background checks (CASC and APS registries) and pass an employment background check; E-Verify compliance for new state employees.
