As a Home Health PT at Inspired Homecare, you will have the opportunity to review patient information to tailor your assessment and treatment plans effectively. Your role includes conducting detailed functional assessments and identifying home/vocational and equipment needs. By developing personalized treatment plans with achievable goals, you'll make a meaningful impact on our clients' lives. Complying with evaluation and documentation guidelines, you'll maintain accurate records to ensure seamless care delivery.

Additionally, you'll play a vital role in instructing and supervising Assistants to guarantee the highest quality of care.

Your progress notes following each treatment session will track the client's journey to improved health and well-being.

Knowledge and skills required for the position are:

• Current PT License for the State of AZ
• AZ fingerprint card
• Valid CPR Certification
• Driver's License with valid insurance
• Experience with EMR system HCHB; preferred but not required
