INSPIRED home health care agency is rapidly expanding, and we are hiring effective immediately. We are looking for a committed Physical Therapist to act as the patient's partner throughout the trip of restoring motion and ensuring that the client will function at their personal best. You will maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation.
Currently offering salary of120,00 annually, or pay per visit rate:
We are needing therapist to service West Valley, Paradise Valley, North Scottsdale, Central Phoenix, Peoria and Glendale
- Conduct initial OASIS evaluations and assessments of patients in their homes to determine their physical therapy needs.
- Review medical histories, diagnoses, and physician orders.
- Identify patient-specific goals and develop individualized treatment plans based on assessments and appropriate service utilization offering cost-effective treatments that help improve clients' motion and mobility
- Reduce the need for medications and provide alternatives to surgery
- Develop care plans using a variety of treatment techniques
- Create fitness- and wellness-oriented programs tailored to patients' specific needs
- Provide quality, personalized and evidence-based care and proven interventions
- Motivate patients during treatment in order to help them function optimally
- Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination
- Consult and practice with other health professionals
- Evaluate effects and monitor and communicate progress
- Document patient care services
- Develop and implement treatment plans to help patients regain mobility, strength, and functionality.
- Educate patients and their families on exercises, techniques, and safety measures to assist in the healing process.
- Utilize various physical therapy modalities (e.g., manual therapy, exercises, therapeutic equipment, etc.).
- Monitor and document patients' progress, adjusting treatment plans as necessary.
- Communicate regularly with physicians and other healthcare professionals to report on patient progress and updates.
- Teach patients and caregivers techniques for proper body mechanics, injury prevention, and at-home exercise programs.
- Provide guidance on the safe use of adaptive devices or equipment, such as walkers or braces.
- Work closely with other healthcare team members, including physicians, occupational therapists, nurses, and social workers, to ensure comprehensive care.
- Attend multidisciplinary meetings and provide input on patient care plans.
- Maintain accurate, up-to-date patient records in compliance with HIPAA regulations and company policies.
- Complete all required documentation for billing, insurance, and patient records.
- Travel to patients' homes to deliver services, which may include driving long distances or managing a caseload.
