Occupational Therapy (OT) East Valley supports clients in their homes by delivering skilled, individualized occupational therapy that improves daily function, safety, and independence. This role exists to turn clinical findings into clear, measurable progress.

Key responsibilities include completing evaluations and re-evaluations; creating and updating individualized plans of care; providing therapeutic interventions and caregiver education; coordinating with the interdisciplinary team to maintain continuity; and completing accurate, timely documentation of visits, outcomes, and communication.

• Current Arizona Occupational Therapist license in good standing
• Home health or comparable community-based experience preferred
• Strong clinical judgment, organization, and documentation accuracy
• Clear communication and ability to work independently

Your Occupational Therapy (OT) East Valley day is organized around planned home visits across the East Valley. You start by reviewing your schedule, confirming addresses and priorities, and mapping efficient travel between clients. In the field, you focus on focused, one-on-one time with each client in their home environment, then transition between visits with brief check-ins and note completion. Throughout the day, you stay aligned with the care team through timely updates and clarifications so next steps remain clear.

You wrap up by ensuring all records are submitted and the next day's visit plan is set.
