* Starting annual salary of $100,000 for New Grads, up to $140,000 annual salary depending on experience!

Key Responsibilities Develop and implement individualized care plans

Provide therapy services per physician’s orders

Assess functional status and recommend adaptive equipment

Teach new skills or retrain patients in daily activities

Maintain accurate documentation and participate in case conferences

Requirements

Licensed Occupational Therapist (OT) in the State of Arizona Graduate of an ACOTE-accredited OT program, eligible for NBCOT certification

Current CPR certification

Valid driver’s license, insurance, and reliable transportation
