Behavioral Health Connections (BHC) in Tempe, AZ is seeking a dynamic and talented LPN Referral Management Full Time Nights to work 12 hours Shifts. LPN serves to assess and coordinate the care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs.

This role is crucial in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.

Job Duties /Responsibilities:

* Review assessment referrals received and collaborate with RN to assess patient needs.

* Assist with coordination of patient care

* Maintain patient records, updating databases, and ensuring accurate documentation of patient information

Qualifications

EDUCATION

Nursing Degree, preferred Bachelors or Masters

LICENSURE/CERTIFICATION

Licensed LPN in current State or compact license

Current CPR, if required by a customer hospital. ("Customer hospital" means the hospital to which BHC is contracted to provide mobile assessment services.)

EXPERIENCE

Preferred 1-3 years related work experience in Behavioral Health
