Field‑based coverage includes Maricopa County – Mesa, Gilbert, Tempe, and Chandler – where we provide primary care to seniors in senior living communities and skilled nursing facilities.
Responsibilities- Provide primary care to an assigned panel of members at their homes or facilities; conduct physical/wellness exams and chronic disease management.
- Perform urgent medical visits, telephonic triage, administer vaccines, order and interpret diagnostic tests.
- Initiate goals‑of‑care conversations with patients, families, and community staff.
- Develop comprehensive care plans, collaborate with case‑management teams, and coordinate services with physicians, specialists, hospitals, and community organizations.
- Manage visitation schedules according to practice protocols and adjust them based on acuity.
- Engage with primary care physicians and other health professionals to prevent disease, promote health, and manage chronic conditions.
- Monitor and address clinical quality gaps, document visits and interactions promptly, and act as a direct resource for members and their support staff.
- Participate in Integrated Care Sales events and initiate virtual visits to reduce drive time and enhance triage.
- Serve on the on‑call program after hours and weekends.
- Maintain professional knowledge through workshops, publications, and continuous education, contributing to quality goals and workflow optimization.
- Master’s degree in Nurse Practitioner or an accredited Physician Assistant program.
- Education in adult/geriatric medical care (preferred).
- Current Arizona Nurse Practitioner license or Arizona Physician Assistant Certification (PA‑C) in good standing.
- Wound Care Certificate and Palliative Care Certificate are a plus.
- Knowledge of complex chronic diseases (dementia, diabetes, heart failure, COPD) and their treatment.
- Proficiency with HCC and ICD‑10 coding and documentation.
- Understanding of CMS guidelines and Medicare Advantage Managed Care.
- Ability to initiate goals‑of‑care and end‑of‑life discussions.
- Strong interpersonal skills, change‑agent mindset, and ability to build relationships with teams and patients.
- Knowledge of HIPAA requirements.
- Basic technical skills (Word, Excel, PowerPoint, Outlook, EMR) with openness to new technology.
- Physical capability to stoop, kneel, crouch, bend, reach, lift, pull, push, and carry up to 10lbs regularly and up to 50lbs occasionally.
- Multilingual/multicultural ability is a plus.
- Ability to travel 75+% of the time within the assigned area; valid driver’s license, automobile insurance, and reliable transportation required.
Job offer contingent on proof of recent Tuberculosis screening. Background check required. The position is patient‑facing and may involve differential pay.
