Nurse Practitioner or Physician Assistant

Job Description

The Nurse Practitioner/Physician Assistant for Primary and Palliative Care is responsible for providing relief from the symptoms of patients’ chronic progressive illnesses and preventing hospitalization through triage and coordination of care.

• Provide advanced primary and palliative care to patients at their residence
• Develop treatment plans to provide symptom management
• Provide on-call support and work closely with triage team to prevent hospitalizations and provide patient support
• Collaborate with health care partners and referral sources to promote and sustain community relationships
• Coordinate with patient’s care team and referral sources to improve patient care and prevent readmissions
• Arrange for comfort measures such as pain management, home health and spiritual care
• Assist in end-of-life discussions and hospice transitions if necessary

Various hours M-F flexible

• Master’s degree in nursing from an accredited program with current license to practice professional nursing or graduate from an ARC-PA accredited physician assistant program
• Board certified advanced nurse practitioner or Physician Assistant
• Active state license in good standing
• Current DEA
• CPR certification
• Minimum of 2 years of health care experience in hospital, medical outpatient clinic, home health, hospice or specialty care setting
• Must be a licensed driver with an automobile that is in good working order and demonstrate ongoing insurance in accordance with state and or organization requirements
