Registered Nurse – Home Health Client

As a Registered Nurse for our home health client, you will provide skilled nursing care to patients in their homes. You will work closely with the interdisciplinary team to develop and implement individualized care plans, monitor patient progress, and provide education and support to patients and their families. This is an onsite position based in Tempe, AZ.

• Conduct initial assessments and ongoing evaluations of patient health status
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Administer medications, treatments, and procedures as prescribed by the physician
• Monitor patient progress and make necessary adjustments to care plans
• Provide education and support to patients and their families on disease management, medication management, and other aspects of care
• Document all patient care activities accurately and timely
• Communicate with physicians and other healthcare professionals to coordinate care and ensure quality outcomes
• Maintain a safe and clean environment for patients and their families
• Adhere to all company policies, procedures, and regulations

• Current and unrestricted Registered Nurse license in the state of Arizona
• Minimum of 1 year of nursing experience, preferably in home health or a related field
• Strong clinical skills and knowledge of home health regulations and standards of practice
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

• Must be able to travel to patients' homes within the Tempe, AZ area
