Position
An exciting opportunity to join a value‑based care organization based in Arizona. The practice is seeking a board‑certified Nurse Practitioner in Primary Care/Adult Medicine or Geriatrics experience to join their team.
ScheduleMonday – Friday. Traveling role seeing patients in their homes. No nights, no weekends.
CompensationCompetitive base salary, full benefits, PTO, CME time plus allowance, and a sign‑on bonus.
Responsibilities and Qualifications- Provide nurse practitioner‑level primary care and geriatric care in an outpatient and home visiting setting.
- Support physicians and interdisciplinary staff in value‑based care, preventive medicine and chronic disease management.
- Manage patient panel and maintain continuity of care within structured workflows.
- Travel to patient homes as required.
- Board‑certified in Primary Care/Adult Medicine or Geriatrics; experience in geriatric population preferred but not required.
