Behavioral Health Connections (BHC) in Tempe, AZ is seeking a dynamic referral management RN to work full‑time nights, 12‑hour shifts, under the direction of the Clinical Program Director. The RN will manage electronic referrals, coordinate care with sending facilities, patients, and families, and conduct thorough clinical reviews of medical records. The role involves collaborating with healthcare providers to ensure safe patient placements and compliance with HR policies and HIPAA standards, providing basic insurance benefits knowledge, supporting the crisis assessor team, and conducting crisis assessments as needed.
- Review assessment referrals received from various sources to assess patient needs.
- Assist in coordinating patient care.
- Maintain patient records, update databases, and ensure accurate documentation in approved EMR and other platforms.

Education
:
Degree in Nursing, preferred Bachelor’s or Master’s.
Licensure/Certification
:
- Required:
Licensed RN with current state nursing license. - Preferred:
Nursing Licensure Compact. - Current CPR and vaccinations, if required by the customer hospital.
Experience
: 1–3 years work experience in behavioral health.
