Referral Management RN (PRN)

Behavioral Health Connections (BHC) in Tempe, AZ is seeking a dynamic and talented Referral Management RN to work 12‑hour shifts. Under the direction of the Clinical Program Director, the RN will manage electronic referrals, coordinate care with sending facilities, patients, and families, and conduct thorough clinical reviews of medical records. The position involves collaborating with healthcare providers to ensure safe patient placements and compliance with HR policies and HIPAA standards, and providing support to the crisis assessor team with crisis assessments as needed.

• Review assessment referrals received from various sources in collaboration with the RN and other clinicians to assess patient needs.
• Assist in coordinating patient care.
• Maintain patient records, update databases, and ensure accurate documentation in approved EMR and other platforms.

• Degree in Nursing:
  Bachelors or Masters preferred.
• Licensed RN – current state nursing license required;
  Nursing Licensure Compact preferred.
• Current CPR certification; vaccines as required by customer hospital.
• 1‑3 years experience in behavioral health.
